The Self care team is based at NHS Kirklees, Broadlea House, Dyson Wood Way, Bradley, Huddersfield, HD2 1GZ. You can telephone the Self care team on 01484 464 443 who will be able to advise you on the services available in your area.

Health trainers

Health trainers can provide you with a structured one to one session to support and motivate you in managing your condition. As part of your support you will be provided with details of all the local activities and support groups in your local area to help you feel more supported with your condition. Your health trainer will help you to set and achieve your goals through helping you with your motivation, taking control and living with your condition and developing a plan of action tailored to your needs.  Contact the Health Trainer Team on 01484 414 933.

The Expert Patient Programme (EPP)

The Expert Patient Programme is for anyone with a long term health condition or illness who wishes to learn skills for controlling and improving their health. The courses are held locally and are tailored to the needs of the people who attend. If you would like to find out more about the Expert Patient Programme, please call 01484  464 440.  The Expert Patients Programme Team will be able to provide you with infomation about the programme and availability of courses in your area.
