Weight management Service Prize Draw Terms and Conditions

1st prize:  A £50 high street voucher. The voucher is a ‘One4all’ gift card which can be redeemed at many of the UKs leading retailers. See One4allgiftcard.co.uk for the full list of retailers.

2nd Prize: A complimentary family ticket (2 adults and 2 children) to a Huddersfield Giants home game in the engage Super League 2011 season.

Upon the completion and submission of the weight management service questionnaire, you have the opportunity to win a either a £50 high street voucher or, a family ticket to watch Huddersfield Giants. The following terms and conditions apply:
 
Prize draw rules
 
1. The draw is open to all UK residents aged 18 and over, except employees, and the immediate families of employees, of NHS Kirklees, their agents or anyone professionally connected with this prize draw.
2. The closing date for the receipt of all entries is Monday 27th June 2011.
3. Only one entry per person is permitted. No third-party entries.
4. No responsibility can be accepted for entries that are incomplete
5. The prize must be accepted as offered. There is no cash alternative.
6. The winner will be notified by their preferred method of contact within 14 days of the closing date.
7. The promoter’s decision is final and binding. The winner will be the first entry drawn by an independent person.
8. Entry implies acceptance of these rules.
9. NHS Kirklees reserves the right to substitute a prize of equal or greater value should circumstances make this necessary. If the prize is not claimed within three months, a re-draw will take place.
10. Winners may be required to take part in publicity resulting from this prize draw.
