Referrals

How can you refer to this programme?

Referral onto this programme can be arranged following completion of the Expert Patients Programme course. The Admin and Support Officer will write to the GP to get a referral for the Get Food Wise and Exercise Programme.

Once this programme has been completed, the patient can go on to attend the full PALS programme with no extra referral needed.
