Referral information

All patients referred to the Heart Failure Nurse Specialist Service will have had their diagnosis of heart failure confirmed by Echocardiogram or Angiogram.

Referral can be made by completing the referral form but if in doubt please contact the heart failure nurses.

If you are unsure if the patient fulfils the criteria for referral, please contact the HFNS and discuss the individual case.
