Referrals

Referral is via the GP or other health professional - please see the Referral Pathway.

Patient consent is required to generate an assessment by the Pulmonary Rehabilitation team to make sure the programme is suitable for the patient.

For referral forms, please call the Pulmonary Rehabilitation Team Secretary on 01924 512075. A proper form must be used as these are in triplicate print and photocopies cannot be accepted.
