How can you refer to this programme?

Participants can self-refer by completing a registration form that can be found in the EPP leaflet. The leaflets are widely distributed across GP Practices, Health Centres, community venues and public buildings e.g Job Centre Plus and Social Services Information Points.

Health professionals can also complete the form on behalf of the patient and send to the following address:
